I have been a resident at Sonoma post acute and I have been treated well, I really enjoyed the staff and care I received during my rehabilitation. The therapy team was very nice and helpful, social services has been great at scheduling my appointments and the nursing staff has shown great care. Would recommend.

Our family is incredibly grateful for the exceptional care our 84-year-old mother received while recovering from a broken hip at this facility. From the very beginning, the admissions department went above and beyond to help coordinate what turned out to be a complicated transfer from an acute care facility. Their professionalism, persistence, and compassion made a stressful situation much easier for our family to navigate. We are especially thankful for their efforts in ensuring a smooth transition and helping get our mother the care she needed. Throughout her stay, the nursing staff, therapists, and support personnel treated her with kindness, dignity, and respect. Recovering from a hip fracture at her age was no small task, but the team encouraged her every step of the way and helped her regain strength, mobility, and confidence. A special thank you to the physical and occupational therapy teams. Their skill, dedication, and genuine concern for her well-being played a vital role in her recovery. They kept us informed, answered our questions, and celebrated every milestone with her. Entrusting the care of a loved one to others is never easy, but this team gave us peace of mind knowing she was in capable and caring hands. Thanks to their hard work and commitment, our mother was able to return home stronger and more independent. We will always be thankful for the outstanding care and support our family received.

Such a nice building and helpful staff. Katie the Director of Marketing is incredibly helpful and knowledgeable about the care that can be provided. Her expertise and easy communication are noticed. Definitely recommend this facility for those seeking Skilled Nursing in Sonoma.

Sonoma Post Acute was wonderful, Katie made the admission process easy and answered all our questions, there was a lot! The therapy team was great and got grandma back on her feet in no time!
