I would like to start by thanking the staff at Sonoma Post Acute for taking such great care of my mom. She was a patient there for quite some time before transitioning to an assisted living facility. A special thank you to the therapy department for working so hard to help her return to her baseline. Your dedication truly made a difference. I would also like to recognize the social worker, JC, who was wonderful throughout the transition to assisted living. They made the process easy by communicating every step of the way, arranging home health services, helping find a new primary care provider, and ordering necessary medical equipment. Thank you as well to the nursing staff and CNAs for the compassionate care you provided to my mom. We are very grateful to Sonoma Post Acute for providing such excellent care.

This place is a bit outdated with some broken furniture and mediocre service. Some staff and nurses are great, and some not so much. People end up here because they are unable to care for themselves. When they are starved or left sitting in a soiled diaper for hours on end it makes them feel undignified and forgotten. These are our parents, grandparents, and veterans being left to rot in their beds unless they are able to move on their own. Are you understaffed? What is the issue here? Why are there so many uncaring employees, forcing the few good ones to pick up their slack? Executives, you should eat here every day to experience what you are feeding the disabled elderly. Leave a review on the quality of the slop you serve up. I bet you won’t eat it.

I went to visit my friend today. Her clothes were really dirty and it was evident that she had not been showered in several days. She received no help maneuvering her wheel chair to the bathroom and no care whatsoever during the several hours I was there. I’m saddened that people have to live like this.

I am a friend or relative of a current/past resident The facility is older, but kept fairly clean. 99% of the CNAs, RNs, therapists, and helpers are nice and want to help the patient. Food was for calories, not necessarily nutrition, decent. The main issue we had is with the administration. The person we had to speak to about coordinating therapy and care, release dates etc., is not very good from the patient standpoint (this is based on our experience). At one point we literally had asked 4 to 6 different people over the course of 5 days to notify this person that we wanted to speak to them. They just never showed up. This continued throughout my husband’s stay. My husband has been home for 3 months and we just got a call about the in-home equipment we were told was ordered when he was discharged. We also got a 10 day runaround from their business office where we had to constantly call them, again, they just wouldn’t call us back. I would not recommend this place based on our experience. The stay was stressful enough without dealing with the added stress of upper management and admin.

Awful! My mother has had to sit in her dirty diaper for ever! After calling the nurse a million times she finally came after 30 min. I had to change her, she had no wipes. The one nice thing I can say is there are a few lovely nurses (that you rarely see) and the social worker is wonderful.

JC was absolutely amazing in helping coordinate the move-in for my new resident. He was so caring, patient, and professional throughout the entire process. JC made sure all documents were completed properly and went the extra mile to ensure everything went smoothly. His attention to detail and genuine compassion made a stressful process feel effortless. I am/was a resident of this facility I was at Sonoma Post Acute recuperating from a hip surgery. Nothing was really good. It was probably the poorest of any of the places I’ve stayed at. There was a lack of help. The toilet was always leaking and I never got it fixed. As far as coming to your attention or whatever, they mostly had young CNAs that didn’t know that much. The staff was always short. They were shorthanded on therapy people. They kept losing them. I hardly ever got any therapy as far as my hip. They were doing things like sitting in the chair, lifting your hands up, etc. to keep the therapy going, but not concentrating on the things that need to be done. It was not a very good experience. I would never go back there again. The senior staff was very good, but the younger ones weren’t good. It’s not their fault, but just a lack of experience. The food was OK. You just didn’t get it. They were always out of stuff.

I love Sonoma Post Acute, and would confidently recommend them to anyone looking for skilled nursing support in Sonoma County. Katie and the Sonoma Post Acute team were so helpful in getting one of my clients admitted and provided excellent care throughout their stay. People often don’t realize all that goes into getting someone admitted to a Skilled Nursing Community, and it’s quite the surprise process! Particularly when one is dealing with so much. The community is so clean and comfortable, and provides excellent care. They happily participated in therapy and left for home feeling better than when they entered the hospital. The entire team truly cares about their patients and their families, and I’ll continue working with them for years to come.

5 Stars for Sonoma Post Acute Skilled Nursing & Katie Barron! I had an excellent experience with Sonoma Post Acute Skilled Nursing and I can’t thank them enough for the exceptional care they provide. The staff is not only highly skilled but also compassionate, ensuring that my loved one received top-notch care in a comfortable environment. They took the time to truly understand and meet their needs, which made a world of difference in their recovery. A special shout-out to Katie Barron, the marketer, who went above and beyond to answer all my questions, provide helpful information, and make the transition as smooth as possible. Katie’s professionalism and genuine care for patients and families are evident, and I truly appreciate all of her efforts. Went to visit my 92 year old mother in law after a fall and hospital visit.The desk staff were so involved in the phones they almost didn’t look up. Asked which room she was in and they said “Is it a man?” Then; “ I don’t think she’s here.” Eventually they said she was there. Before we arrived, my mother in law had to get out of bed, go to the door with her walker and and shout for someone to assist her. While were visiting she wanted some juice, I used the call button and no one responded for and hour and a half. She had been assisted earlier to get to the bathroom but the nurse had left before she’d finished and she had to make her son was back to the bed. When making calls the the front desk, no one answered. The level of “care” here is sub par to say the least, the rooms are dingy and in need of repair. The staff are lackluster and trying to obtain information regarding doctor visits is almost impossible.

Very impressed with the therapy and nursing care here. They were able to rehab my mom and take care of all of her wound care while getting her home in a timely manner. They worked with me on making sure her home was safe her to go back to. I’m very impressed to say the least.

I wasnt the easiest patient but this place is beyond great. Therapy was amazing and i almost didnt want to leave. Overall great expirence.

The staff go above and beyond to make this wellness center feel like home. The staff is attentive, kind and most importantly supportive. I would recommend this facility to everyone

The staff is very helpful and Considerate. The Therapists are all top notch and work overtime to make sure the patients are satisfied with the results of the therapy that is performed. Their food is of excellent quality and very yummy. Snacks are given whenever requested. As a patient I highly recommend the Sonoma Post Acute Care Facility to anyone who appreciates being well taken care of.
